Guideline on how to submit an International Veterinary Certificate/Movement permit
Once login/registered
To submit a new application, click on the “New application” button on top-right of the screen.
Kindly note that:

First time applicants are requested to register on the Portal by submitting the required information OR contact their registered veterinary surgeon (treating veterinarian) to apply on their behalf.

Applicants can afterwards view the progress of his application from the dashboard or from dashboard of selected registered veterinary surgeon.
In the new application page
Consignor
Name and full address of the natural or legal person dispatching the consignment.
Information on telephone and fax numbers or e-mail address is recommended.
Consignee
Name and full address of the natural or legal person to whom the consignment is destined at the time the certificate is issued.
Kindly note that all fields are mandatory.
Country of origin
Name of the country from which the commodities are being exported.
Kindly note that Full Name, City and Country are mandatory fields.
Details on the nature of the commodity sufficient to identify it
For animals and hatching eggs, please fill in the fields:
Species (scientific name); Identification system (The identification should be by a permanent marking. For e.g. Identification number or identification details such as tattoo/close ring/shell number); Identification number or other identification details; Quantity and if required, Breed / Category (e.g. heifer, steer, layer, broiler); Age; Sex.
For other commodities, please fill in the fields:
means of conveyance (select either sea or air), Purpose (select either commercial or non-commercial), Country of origin (select from drop down list), Country of destination (select from drop down list), Point of Entry (select from drop down list), Expected Departure Date (select date from calendar), CITES and Import Permit Number if applicable.
Total quantity of the commodity
For animals, hatching eggs and animal products, provide with the total count of animals, eggs.
For products, provide with the gross weight and the net weight in kg of the whole consignment.
Select units of measure (Eg: kg, units)
Select registered veterinary surgeon (if applicable)
Documents
Applicant may wish to upload any documents in this section by clicking on the drag and drop icon and select your documents you wish to add.

Submit your Application
Click on submit button to submit your application for processing by the LVD.
Applicant can afterwards view the progress of his application from the dashboard or from the dashboard of selected registered veterinary surgeon.
